My practice is using In Practice Systems Vision software and intends to do so for the foreseeable future. I agree to supply data to the General Practice Research Database and to participate in the verification services when required.

Verification services play a key role in validating and enhancing public health research. Contributing practices are paid for both data collections and verification services. To proceed with your application, a member of our Collections Teams will contact you to request the first data collection from your practice. You should receive this request in about a month from receipt of this form.
